Care Manager

JOB SUMMARY: Under the immediate supervision of the In-Home Services Director, the employee shall provide outreach services, information and assistance to eligible clients.  The Care Manager shall also be a contact person for DTGC in the field.

JOB QUALIFICATIONS: The Care Manager shall have received a Bachelor’s Degree in Social Work or a related field and two or more years relevant work experience. Proficient computer skills are required. Must hold a valid driver’s license and have "zero points or + points" driving record. Experience working with the aging population preferred.
